ABSTRACT

Objective:To analyze the operation status of official voice in Beijing municipal hospitals and explore the countermeasures to optimize the operation of official short video tiktok.Methods:The 21 Beijing municipal tertiary public hospitals were selected as the research objects. The 17 tiktok numbers which had been approved by the hospital as of December 31, 2020 were included. The data of basic accounts, fans, points, themes, contents, and number of periodicity were analyzed. The theme and the number of praise of the short video tiktok that broke through 10 thousand times were analyzed. The current situation and problems of official voice in Beijing municipal hospitals were analyzed.Results:The 17 official tiktok accounts of the hospital issued 2 059 pieces of tiktok, 1 737.5 thousand fans, and 9 231.8 thousand times of points, the average number of entries was 4.5 thousand times. The official tiktok construction of Beijing municipal hospitals was still in its infancy. The problems included: the weakening of IP image and the lack of system design, lack of content productivity and interaction, unclear positioning and lack of operation strategy.Conclusions:The optimization of hospital official tiktok operation needs to properly handle the relationship between science and interest, systematicness and efficiency. We should clarify the account positioning, improve productivity, strengthen interaction, enhance linkage, and do a good job in supervision.

ABSTRACT

Objective:To observe the therapeutic efficacy and prognosis of daratumumab combined with chemotherapy bridging to allogeneic hematopoietic stem cell transplantation (allo-HSCT) followed by daratumumab and lenalidomide maintenance treatment for primary plasma cell leukemia (PCL).Methods:The clinical data of a patient with primary PCL admitted to the First People's Hospital of Yunnan Province in January 2020 were retrospectively analyzed, and relevant literatures were reviewed.Results:The patient was diagnosed with primary PCL and treated with daratumumab combined with BD (bortezomib + dexamethasone) for 1 course and BCDD (bortezomib + cyclophosphamide + liposomaldoxorubicin + dexamethasone) for two courses. The patient was treated with daratumumab combined with allo-HSCT after complete remission. The donor cells were successfully implanted and the chimerism rate of donor cells was 94.36% without acute graft-versus-host disease reaction. And then the patient received intermittent maintenance therapy of daratumumab combined with low dose lenalidomide after transplantation, and the current remission period after transplantation reached 4 months.Conclusions:Daratumumab combined with chemotherapy bridging to allo-HSCT followed by daratumumab and lenalidomide may improve the prognosis of primary PCL and prolong survival time.

ABSTRACT

Objective: To identify appropriate ways of applying the current cervical screening methods to minimize the occurrence of missed diagnosis of high-grade cervical intraepithelial neoplasia (CIN2/3) and cervical cancer. Methods: A retrospective analysis was conducted based on the clinical data of 318 patients, who had CIN2/3 or cervical cancer and were treated in Aviation General Hospital Affiliated to China Medical University between June 2014 and August 2018. Among these cases, 296 involved CIN2/3, and 22 involved cervical cancer. All of the patients involved underwent a thinprep cytology test (TCT) and high-risk human papilloma virus (hrHPV) screening, and their age and TCT and hrHPV results were further analyzed. Results: Among the 296 patients diagnosed with CIN2/3, 130 of them (43.92%) were aged between 30 and 39 years, ranking the top among all age groups. Sixty-nine young patients (23.31%) aged between 20 and 29 years were diagnosed with CIN2/3, ranking in the third place. Among all of the 318 cases, the TCT results were positive in 199 cases (62.58%), hrHPV results were positive in 308 cases (96.86%), and there were 313 cases (98.43%, 313/318) wherein both TCT and hrHPV results were positive according to joint screening. The most predominant hrHPV subtypes were 16, 52, 58, 33, 18, and 31. Conclusions: As an increasing number of young women are diagnosed with CIN2/3 and cervical cancer, their screening should not be ignored. Single use of TCT screening leads to more frequent missed diagnosis of high-grade lesions compared with single use of hrHPV screening, while joint use of both screening methods could improve the detection rate. Even if TCT results were negative, it is suggested to conduct a colposcopy for those patients with high-risk subtypes besides hrHPV16 and 18 positivity, and in particular, for patients with hrHPV 52,58,33, and 31 positivity.

ABSTRACT

OBJECTIVE@#To analyze the characteristics of vestibular migraine (VM), in order to understand the auditory and vestibular function.@*METHOD@#One hundred cases of confirmed or suspected VM patients were observed. Routine examination including pure tone test, vestibular dual temperature test and vestibular evoked myogenic potential(VEMP) and video head impulse test(v-HIT) Were conducted.@*RESULT@#The incidence of male and female was about 1.00 : 2. 57. The average age of onset was 47 years, of which the youngest was 19 years old, and the oldest was 74 years old. Ninty-three cases of patients presented with vestibular sensitive performance, including photophobia, phonophobia,or motion sensitive. Fourty-two patients had migraine at the onset of the disease. Thirty-four patients had cochiear symptoms, including tinnitus, ear fullness, or epicophosis. In seventeen cases of fatigue, tension or poor sleep can cause vertigo. Nine patients had low blood pressure, 7 patients presented with hypertension. Two cases of vertigo were closely related to the change of position, and 1 case had visual aura. In the experiment, 94 patients had undergone VEMP test,with 44 cases of abnormal VEMP value. Seventy-nine patients had been examined v-HIT,of whom 2 patients were abnormal (both of which were reduced). Pure tone audiometry was abnormal in 35 cases. Five cases of patients showed abnormal vestibular-double temperature test.@*CONCLUSION@#Vestibular migraine is more prone to female, with young middle age, and can be accompanied by cochlear symptoms. Vestibular function tests (low frequency and high frequency) are normal.

[ ABSTRACT] AIM: To investigate the underlying mechanisms responsible for endothelial dysfunction of type 1 diabetes mellitus (DM) rats fed with high-salt diet.METHODS:Type 1 DM was induced by intraperitoneal injection of streptozotocin (70 mg/kg).Normal and diabetic rats were fed high-salt food (HS, 8% NaCl) and standard food for 6 weeks, respectively.Isometric tension of the mesenteric arteries were measured .The expression of Akt , endothelial nitric oxide synthase (eNOS) and caveolin-1 (Cav-1) was examined by Western blot .RESULTS:The rats in DM+HS group exhibited more pronounced impairment of vasorelaxation to acetylcholine and insulin compared with either DM group or HS group (P<0.01).Akt and eNOS phosphorylation levels, and nitric oxide (NO) concentration in DM +HS group were significantly lower than those in DM group (P<0.01).The level of Cav-1 in DM+HS group was significantly higher than that in DM group and HS group .CONCLUSION:Impaired endothelial Akt activation , increased Cav-1 expression and re-sultant decreased eNOS activation contribute to aggravate high-salt diet-induced endothelial dysfunction and hypertension in DM rats.

ABSTRACT

Objective To assess the impacts of management of psychological health on metabolic indictors in community residents.Methods A total of 300 health check-up adults from one single community were randomly assigned to the control group (receiving routine health management,n =150) and the study group (receiving psychological health management,n =150).Blood pressure (BP),fasting plasma glucose (FPG),total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C) and life style were evaluated and compared at baseline and 1-year follow-up.Results No difference of clinical data was found between the two groups at baseline.After 1-year intervention,current sigarette smoking,BP,FPG,TC,TG and LDL-C of the study group were significantly decreased when compared with the control group (x2 =7.54,t =6.43,5.38,5.89,6.15,4.36,6.69,5.47 ; all P < 0.05).Compliance to and satisfaction with health management were significantly improved in the study group (x2 =9.13,12.35,11.96,10.98,9.95 ; all P < 0.05).Conclusion Management of psychological health may be effective in improving healthy life style,patients' satisfaction and cardiovascular metabolism in community residents.

ABSTRACT

Objective To study the difference and clinical significance of serum Angiopoietin-2 (Ang-2) level in gastric cancer, precancerous disease and the changes in post operation follow-up patients.MethodsSerum Ang-2 and CEA levels were detected with ELISA method in 158 gastric cancer patients, 31 chronic astrophic gastritis patients, 38 gastric adenomatous polyp patients, 28 chronic gastritis patients with benign gastric ulcer and 30 healthy controls.The changes of serum Ang2 levels in 131 patients with radical gastrectomy were measured in 2-year post-operational followed-up and the prognosis was also evaluated.The correlation between serum Ang-2 level and the pathologic character of gastric cancer was analyzed.ResultsThe serum Ang-2 and CEA levels of gastric cancer group were (331.8± 64.3) μg/L and (42.6 ± 37.3) μg/L, respectively, which were significantly higher than those of healthy control group [(187.4±32.7) μg/L and (4.2±3.1) μg/L]and benign gastric ulcer group [(197.3±35.4) μg/L and (4.5±3.2) μg/L](all P <0.01).The serum Ang-2 level in chronic astrophic gastritis group was also markedly increased (P<0.05), however there was no significant change in CEA level.There was no obvious change of Ang-2 and CEA level in gastric adenomatous polyp group (P>0.05).Pre-operation serum Ang-2 levels of the relapse group and the metastasis group were significantly higher than that of no relapse group (P<0.05).Post-operation serum Ang-2 level of no relapse group decreased to normal range in 1 month and without obvious increase in 2 years follow-up.The serum Ang-2 level of the relapse group also decreased to normal range in 1 month post-operation, while which was significantly higher than that of no relapse group and normal control group from 6 months to 2 years after operation (P<0.01).Serum Ang-2 level of the metastasis group was markedly higher than that of no relapse group and normal control group during 2 years after operation (P<0.01).The 2-year survival rate of no relapse group was significant higher than that of relapse group and metastasis group.The change of serum Ang-2 level was correlated with in the gastric cancer differentiation degree, TNM pathologic classification, lymphatic metastasis, distant metastasis, the depth of invasion and the tumor size (P<0.01), however not correlated with histopathology type and the location of tumor.ConclusionSerum Ang-2 level may be a new and valuable gastric cancer marker, which will help to monitor tumor relapse, migration and prognosis assessment.

ABSTRACT

Objective To investigate the effects of health management on risk factors of cardiovascular diseases in community residents.Methods A total of 208 adults from one single community who visited our health check-up center were randomly assigned to the control group(receiving routine consultation,n =104)and health management group(receiving health management intervention,n =104).At 1-year follow-up,fasting blood glucose(FBG),lipid profiles,and blood pressure(BP)were measured and risk factors of cardiovascular were surveyed.t or x2 test was used for data analysis.Results No difference of clinical data was found between the two groups at baseline.Follow-up results indicated that absolute risk of cardiovascular diseases,number of risk factors,health satisfaction,sick leave,daily vegetables intake and physical exercise,and BP were significantly improved in the health management group.Ten-year absolute risk of cardiovascular diseases(0.8 ± 0.4 vs 1.3 ± 0.5),number of risk factors(172 vs 283),and sick leave per year(118 d vs 264 d)were significantly decreased in the health management group when compared to the control group(all P < 0.05).Furthermore,health satisfaction(83.7%(87/104)vs 52.9%(55/104)),and daily vegetables intake(520.5 g vs 348.3 g)and physical exercise(1.2 h vs 0.3h)were significantly improved in the health management group(all P < 0.05).Conclusion Health management may be effective in decreasing risk factors of cardiovascular diseases in community residents.

ABSTRACT

Objective: To investigate the psychosocial characteristics and mind-physique status of pa-tients with gynecological malignant tumors. Methods: Clinical psycho-rating scale [life event scale (LES) and symptom checklist-90 (SCL-90)] was applied to test life events and mind-physique status of 60 cases of pa-tients with gynecological malignant tumor. The results were compared with those of 40 female patients with benign tumors and 40 normal females. Results: The frequency and tension value of total life events and those of negative life events in the malignant cancer group were much higher than those in the benign tumor group and normal group (P<0.05) and no remarkable differences were found in the frequency of positive life events among the three groups. The frequencies of total life events and negative life events were higher in the be-nign tumor group than those in the normal group (P<0.05) but there were no significant differences in the fre-quency of positive life events, total tension value or positive tension value among the three groups. No signifi-cant differences were found in the indices of SCL-90, positive scores and scorns of other factors among the three groups (P>0.05). The scores of somatization, obsession, interpersonal relationship sensitivity, anxiety, hostility, phobia, and paranoia were higher in the malignant tumor group than in the benign tumor group and normal group (P<0.05). The average positive scores and the depression scores in the benign tumor group were higher than those in the normal group (P<0.05), but there was no statistical significance in other indices between the benign tumor group and the normal group (P>0.05). Conclusion: Negative psychosocial factors are closely correlated with the occurrence and growth of gynecological malignant tumors. Active psychological intervention should be performed as eady as possible.

ABSTRACT

Objective To decrease the unhappiness of children and make them happy to the utmost in their process of seeking medical help by constructing a clinical nursing model of happiness and natural affection. Methods During the clinical process of setting up a suitable surrounding for children's hospi-tal visit and hospitalization, and finding a pleasant method to meet children's psychological needs, indexes of physiological response pre and post admission, discharge and painful operations including surgeries were recorded. Parameters of psychological reactions such as facial expressions, movement reactions and emo-tions were observed and changes of medical compliance were analyzed. We also evaluated variation of fear, time of disease rehabilitation, the satisfaction degree of parents and doctors. Results Child's fear was lowered obviously, cooperation and compliance of treatment were improved remarkably, time of disease re-habilitation was shortened to different degree, the satisfaction degree of patients and doctors had been im-proved to above 98%. Conclusions The clinical nursing model of happiness and natural affection for children can decrease the unhappiness of children and make them happy to the utmost in their process of seeking medical help.

ABSTRACT

<p><b>BACKGROUND</b>Malignant pleural effusion is usually caused by lung cancer, and tumor markers may be helpful to its differential diagnosis. The aim of this study is to explore the clinical value of serum and pleural effusion pro-gastrin-releasing peptide (ProGRP), neuron specific enolase (NSE), cyto- keratin fragment 19 (CYFRA21-1) and carcinoembryonic antigen (CEA) in differential diagnosis and histological typing of malignant pleural effusion caused by lung cancer.</p><p><b>METHODS</b>According to histological type of primary tumor, 99 patients with malignant pleural effusion caused by lung cancer were divided into small cell lung cancer (SCLC) group, adenocarcinoma group and squamous cell carcinoma group, with 37 patients with benign pleural effusion and 35 healthy persons as controls. Diagnostic value of serum and pleural effusion ProGRP , NSE, CYFRA21-1 and CEA was evaluated for each group.</p><p><b>RESULTS</b>The levels of ProGRP, NSE, CYFRA21-1 and CEA in serum and pleural effusion of all the malignant groups were significantly higher than those in the control groups (P < 0.01). In the SCLC group, detection of pleural effusion ProGRP showed the highest Youden index and accuracy. In the adenocarcinoma group and squamous cell carcinoma group, combined detection of pleural effusion CEA+CYFRA21-1 (on parallel test) showed the highest Youden index and accuracy.</p><p><b>CONCLUSIONS</b>Detection of pleural effusion tumor markers ProGRP, CYFRA21-1, NSE and CEA is of great clinical value in differential diagnosis and histological typing of malignant pleural effusion. Pleural effusion ProGRP is the optimal tumor marker for malignant pleural effusion caused by SCLC. Pleural effusion CEA+CYFRA21-1 (on parallel test) is a good auxiliary diagnosis index for malignant pleural effusion caused by adenocarcinoma and squamous cell carcinoma of the lung.</p>
